Complete the Contact Information section to identify your account. We will send you an email to confirm that your changes have been completed.

Voicemail PIN Reset:
Provide the area code & phone number, and employee name to reset your voicemail PIN

Voicemail Box Reset:
Provide the area code & phone number, and employee name. Clicking YES will delete all existing voicemails and reset the greeting to default

EVOFax Change:
Change the email address to where your EVOFAX is delivered. Include the area code with your EVOFax number.

Voicemail - Email Notification:
Change the email address to where your Voicemails are delivered. Include the area code with your User Number.

Add/Remove Lines:
Select your phone type from the drop menu

Enter your MAC address located on the back of your phone.
Where is it? Cisco - Polycom

Enter your Serial Number (Cisco only)
Where is it? Cisco

Select Add or Delete for the appropriate Lines for your phone type from the checkboxes and enter the phone number associated with the line(s).

Use the "Description" box for additional instructions if needed.
